Современные технологии лечения больных доброкачественной гиперплазией предстательной железы – обзор инновационных лазерных систем

Аннотация


С ростом процентной части мужчин пожилого возраста доброкачественная гиперплазия предстательной железы (ДГПЖ) получит широкое распространение и будет одной из первостепенных проблем для системы здравоохранения. Лазерные технологии являются альтернативой трансуретральной резекции в хирургическом лечении ДГПЖ. Недостаточное количество исследований и различия в результатах не позволяют сделать выводы относительно оптимального метода лазерной технологии.

Цель работы – на основании данных современных источников научной информации проанализировать результаты применения лазерных технологий в лечении больных доброкачественной гиперплазией предстательной железы.

Лазерные системы для лечения ДГПЖ предоставляют возможность урологам достичь клинических результатов, сравнимых с трансуретральной резекцией простаты, а количество осложнений значительно меньше. Это малоинвазивные операции, способствующие значительному улучшению мочеиспускания, быстрому и устойчивому устранению симптомов. Результаты сравнительного исследования свидетельствуют о безопасности, эффективности лечения с помощью фотоселективной вапоризации простаты (ФВП) пациентов, страдающих симптомами нижних мочевых путей (CНМП) вследствие ДГПЖ. Катетеризация не является обязательной, но возможна на короткий срок. Уменьшение продолжительности госпитализации при проведении фотоселективной вапоризации с мощностью 120 Вт связано с менее частой ирригацией мочевого пузыря, уменьшением гематурии. После ФВП 120 Вт уровень суммарных периоперационных осложнений был ниже, но для подтверждения клинической целесообразности применения литий-триборатного лазера в хирургическом лечении СНМП, обусловленных ДГПЖ, необходимы длительные исследования.
