Синдром переднесистолического движения створки митрального клапана (SAM – systolic anterior motion of the mitral valve)

Аннотация


Цель работы – анализ данных научной литературы по вопросам этиологии, патогенеза, клинических проявлений и лечения такого редкого синдрома, как SAM без гипертрофической кардиомиопатии.

Проведен анализ научной литературы об особенностях диагностики и лечения синдрома переднесистолического движения створки митрального клапана. Приведены современные классификации. Подробно описаны клапанный и хордальный SAM. Обзор проиллюстрирован результатами собственных наблюдений. Определены предикторы возникновения SAM у больных, подлежащих плановым вмешательствам на створках митрального клапана.

Выводы. Несмотря на то, что SAM остается редким синдромом в реальной клинической практике, он требует знания этиологии, патогенеза, особенностей течения, что позволит чаще устанавливать эту патологию у пациентов после пластики митрального клапана и у больных с гипертрофической кардиомиопатией, а также у лиц с аномалиями хордального, клапанного аппарата сердца, и проводить в каждом конкретном случае дифференцированное консервативное и хирургическое лечение.
